Healthy Aging Coordinator:

• Provides technical assistance to subcontractors and site personnel in food safety, perpetual inventory, record keeping, supportive services, and any area where the meal site is having problems.
• Evaluates subcontractor's performance in delivering nutrition services to
• Works with community groups who directly or indirectly support the senior nutrition
• Trains new site managers and cooks in the senior nutrition program regulations and
• Assists Healthy Aging Director in assigned tasks
• Records and maintains client documentation including intake forms, assessments, case notes, and service deliverables in computer database
• Conducts and advises site managers on nutrition education activities and evidenced based programs

REQUIRED QUALIFICATIONS:

• Bachelor’s degree or associate’s degree related to Human Services and two years of experience working in the areas of aging, disabilities, community health or hospital discharge planning
• License to practice as a registered nurse
• License to practice as a licensed practical nurse and four years of experience working in the areas of aging, disabilities, community health or hospital discharge planning; OR
• Must become trained and/or certified as an Alliance of Information and Referral Specialists (AIRS) in accordance with Code of Iowa 17(14.9).
• Adult/Child Mandatory Reporter within 6 months of
• Must successfully complete the Person-Centered Counseling curriculum provided by Elsevier within 30 days of employment as an options counselor or if already CADER certified must complete the Person-Centered Thinking course provided by Elsevier
