Case Manager - FT (40 hours) to complete screenings and comprehensive assessments, develop care plans and conduct case management services for older adults as assigned in a multi-county area around Carroll.  R.N. currently licensed in Iowa -or- B.A. in Social Work or Human Services field. Three or more years related experience. Ability to communicate effectively orally and in writing, good organizational skills, self-starter, computer skills and ability to work with seniors required. Competitive salary and benefit package.
