Are you or a loved one concerned about staying safe at home? Elderbridge can help. We provide connections to services that will help you stay at home and also provide information about issues that concern you. Are you struggling to provide care for a loved one at home? Our staff is able to give expert advice and caring support for the family care-giver. Elderbridge maintains a library of care giver resources such as books and videos that may be borrowed from at least one library in each county or from each of Elderbridge's offices. Do you live alone and worry about getting the services that you need? Our case management program takes the guesswork out of it. Once eligible for the program, our case managers will stay in contact to assure that you get the services and care that you need. Do you have a problem or a concern that seems overwhelming? Elderbirdge offers guidance as needed for any difficult situation an older adult may face. We will address your concern with respect for your dignity and your wish for privacy. Elderbridge staff and volunteers provide the following services across the 20-county area. Advocacy - Provides information to and works with older adults to resolve issues and complaints. Reviews pending legislation and work at the state and national level to address needed changes in programs, policies and funding that effect older adults. Caregiver Support - Provides to the family caregiver information and assistance. Elderbridge maintains a library of Alzheimer's disease and caregiver resources such as books, videos, and audiotapes that may be borrowed free from one or more libraries in each of Elderbridge's twenty counties or any of the Elderbridge offices. Case Management - Offers a system of coordinated assistance, which ensures an appropriate mix of services for the frail elderly so that the individual has the least restrictive level of care. Activities include assessing needs, developing care plans, arranging services, coordinating the provision of services among providers, follow-up and reassessment as required. Information and Assistance (Information and Referral) - Offers current information on available resources and services and links individuals to the opportunities and services that are available for older adults and caregivers. Medicare Part D - Staff in each office are trained to assist older adults and caregivers with identifying the best options for Medicare Part D (prescription insurance) for that individual. Benefits CheckUp - Staff in each office can also assist older adults and caregivers with identifying needs and print a list of assistance programs that may pay for some of your costs of prescription drugs, health care, utility, and other essential items or services.. Renaissance Newsletter - Provides information about Elderbridge services, Senior Nutrition Program meal menus, and other issues related to aging in its monthly issues to subscribers. S.A.L.T. - Provides informational meetings and programs for older adults designed to protect older individuals, groups and organizations from any type of fraudulent, deceitful, unfair transaction or trade practice. Information is also provided by local and state law enforcement officers to help older people prevent injury and/or loss due to crime.
Elderbridge provides the following services by subcontracting with specialized, local providers. Adult Day Care/Adult Day Health - Provides personal care for dependent adults in a supervised, protective, congregate setting during some portion of a 24-hour day. Services offered typically include social and recreational activities, counseling, meals, transportation, rehabilitation, and medication assistance. Case Management - Offers a system of coordinated assistance, which ensures an appropriate mix of services for the frail elderly so that the individual has the least restrictive level of care. Activities include assessing needs, developing care plans, arranging services, coordinating the provision of services among providers, follow-up and reassessment as required. Chore - Provides assistance to persons having difficulty with one or more of the following instrumental activities of daily living: heavy housework, yard work, or sidewalk maintenance. Congregate Meals and Home-delivered Meals - Serves one hot meal that assures a minimum of one-third of the recommended daily intake for older individuals at a senior center, dining center or a diner's home. Counseling - Uses interviews, discussion, or lending a sympathetic ear to advise and enable the older person and family to resolve problems (concrete and emotional) or to relieve temporary stresses encountered. May be done on a one-to-one basis or on a group basis and may be conducted by paid, donated or volunteer staff. Homemaker - Provides assistance to persons having difficulty with one or more of the following instrumental activities of daily living: preparing meals, shopping for personal items, managing money, using the telephone, or doing light housework. Home Repair - Provides assistance in improving or maintaining a residence. Legal Assistance - Provides legal advice, community education, counseling, and representation by an attorney or other person acting under the supervision of an attorney. Medication Management - Review of a person's medication to assess interactions and/or the setup of medications by a pharmacist or nurse that results in assisting a person to remain at home. Personal Care - Provides personal assistance, stand-by assistance, supervision, or cues for persons having difficulties with one or more of the following activities of daily living: eating, dressing, bathing, toileting, and transferring in and out of bed. Protective Payee - Provides financial management for individuals who, at least temporarily, are unable to manage their federal government benefit funds in a manner that preserves the most independence and decision-making power for that individual, while ensuring that the person's basic needs continue to be met. Respite Care - Provides brief periods of relief or rest for family members or other caregivers of older dependent adults. Transportation - Provides a means of transportation for a person who requires help in going from one location to another.
